Our main office policy will always be to treat our patients with professionalism and respect while providing a warm and welcoming environment where patients will receive the highest quality of care. Our goal is for every patient to leave our office with peace-of-mind about the care they have received and a genuine willingness to recommend Dr. Cox to their friends and family as a primary care physician.

Appointments

Dr. Cox is currently accepting new patients. Please call our office at 478-988-0022 to schedule an appointment. Once you make an appointment, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, please call us to reschedule as soon as possible.

What to bring to your First Appointment

To provide you with prompt and thorough treatment, please be sure to bring the following when you visit our practice for your initial visit:

  • Insurance Information / Card
  • Photo Identification
  • Any medical records pertaining to your visit
  • All medications, prescriptions, over-the-counter or herbal supplements you are currently taking (bring bottles)

Please arrive 15 to 20 minutes before your scheduled appointment time to allow for completion of proper registration paperwork before you see Dr. Cox. If you would like to complete your paperwork prior to your appointment, you may download the appropriate forms below.

Insurance

We accept every major health insurance and managed care plan. Please familiarize yourself with your insurance plan, paying close attention to co-pays, referral requirements, emergency care, participating hospitals, pharmacies, and diagnostic imaging facilities.

Coverage limitations are dependent on individual group contracts. Please contact your insurance carrier directly for specific questions about your insurance coverage.

Payment

Payment is expected at the time services are rendered, unless other arrangements have been made. This includes any co-payments or deductibles (amount not covered by your insurance).

For your convenience, our office accepts cash, Visa, MasterCard and Discover.

Forms

Below you will find our new patient form. Please feel free to print and complete the form before your first visit with us. While we do not require that you complete this paperwork prior to visiting our practice, this will save you and our staff time when you come in for your appointment.

Prescription Refills

Requests for prescription refills need to be made during routine office hours and will generally be completed within 24-48 hours. Refill requests will not be taken after regular business hours. Please request prescription refills 2 weeks prior to finishing your medication.
